The “Confidential Information” and “Medical Information Form” should be completed prior to the first therapy session. Please print, complete, and bring these two forms when you come in for your first appointment. Or, if you prefer, you may fill them out in person when you arrive at the office. The “Agreement for Services” and “HIPAA Notice of Privacy Practices” are also posted below for you to review.
